Overview

Gentiva is seeking a Registered Nurse (RN) for hospice services in Newport News, Virginia. The role centers on delivering personalized, compassionate hospice and palliative care to patients and families in their homes or institutions.

Responsibilities
  • Serve as the primary contact for patients and families, guiding them through the hospice journey.
  • Deliver skilled nursing care and document it as outlined in each patient’s individualized Plan of Care, whether at home or assisted living.
  • Perform initial and ongoing assessments, monitor patient condition, and adjust care plans as needed.
  • Collaborate with an interdisciplinary team—including physicians, LPNs, CNAs, social workers, chaplains—to develop and refine care plans.
  • Provide emotional, physical, and spiritual support to patients and families, offering education and bereavement guidance.
  • Educate caregivers on safe care techniques, medication administration, nutrition, and self-care, providing both practical training and emotional reassurance.
  • Communicate changes in patient condition to the care team to ensure timely interventions.
  • Maintain accurate clinical documentation and adhere to infection control and compliance standards.
  • Support quality improvement and scheduling initiatives to deliver consistent, efficient care.
  • Help patients and families understand medication administration, hospice philosophy and services, symptom and pain management, and end-of-life processes and expectations.
Qualifications
  • Education:

    Registered Nurse.
  • Experience:

    Minimum 1 year as an RN in acute care, oncology, long-term care, hospice, or home health (hospice or palliative care experience preferred).
  • Licenses &

    Certifications:

    Current nursing license in the state of employment, CPR certification, valid driver’s license, and current automobile insurance.
  • Knowledge &

    Skills:

    Understanding of terminally ill patients and families, hospice services and multidisciplinary roles, patient assessment, community resources, oral and written communication, documentation, nursing practice, managed care principles, regulatory guidelines (Medicare, Medicaid, ACHC).
  • Personal Traits:
    Sensitivity to life-death issues, tactful communication with diverse populations, flexibility and adaptability, teamwork, and professionalism.
  • Working Conditions:

    Patient homes, institutional settings, and travel by automobile; potential exposure to infections, hazardous materials, and bodily fluids.
