** Hiring Range**$81,000.00 - $/Annual Actual pay is determined based on job-related factors such as relevant experience, education, credentials, skills, internal equity, and business needs.
** Overview
* * The Stroke Nurse Navigator works in collaboration with the interdisciplinary team to compassionately serve and coordinate referrals and help identify high risk patients who might benefit from palliative care or hospice services. In collaboration with the team, the navigator is responsible to organize resources for timely response and support, coordinate attendance at appropriate IDT or other meetings where appropriate patients might be identified and maintain a dashboard that reflects key indicators for this service.

** What you will do
*** Demonstrate knowledge of, and appropriately refer patients to the stroke clinic, and to other community resources for support and assistance. Provide individualized patient education, assess patient's knowledge and barriers about their disease, and follow up with patient/caregivers after discharge.
* Assess patients and families to determine their needs and available resources. Collaborate with case management to ensure timely and appropriate discharge planning needs are arranged. Assess each stroke patient’s record and perform a utilization review.
* Maintain and implement a patient navigation program tailored to the individualized needs of the stroke patient
* Work effectively and harmoniously with the stroke team and other multidisciplinary team members
* Coordinate appointment scheduling and collaborate with case management to ensure quality patient care. Educate patient regarding protocol for tests/appointments.
* Process physician and patient requests for tests and referrals in a timely manner. Respond appropriately and timely to patient calls.
* Work to continually develop and improve the stroke program. Develop robust relationships with the stroke patients and stroke team, works with Ad Hoc Committees. Works with providers and community leaders to assess resource and program needs to improve quality care across the continuum.
* Develop and maintain relationships with key referring and practicing physicians for the identified stroke population and continually work with physicians for care coordination
* Actively work with the stroke team to identify, plan, promote, and implement appropriate community education and awareness.
* Demonstrate strong communication and educational skills, with the ability to work with diverse populations of patients and caregivers.

** Licenses and Certifications
*** Registered Nurse (RN) - Virginia Department of Health Professions (VDHP) (Required)
* CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) (Required)
* NIH Stroke Scale - Test Group A (NIH) - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) (Required)
