We re looking for a dedicated and empathetic Nurse Practitioner (Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner) to join a collaborative team serving residents in long‑term care and skilled nursing settings.

• Evaluate, diagnose, and manage the ongoing health needs of residents in a long‑term care or skilled nursing facility
• Collaborate with an interdisciplinary care team to develop and adjust individualized treatment plans
• Provide compassionate, person‑centered care that improves quality of life for seniors
• Maintain clear, timely documentation and communicate effectively with facility staff and family members

• Active Nurse Practitioner license (APRN) FNP or AGNP
• Experience in geriatrics, long‑term care, or primary care preferred (new graduates encouraged to apply)
• Commitment to patient‑centered care and strong communication skills
• Ability to work independently and as part of a supportive care team

• Salary range: $100,000 $ 120,000
