Overview
The Lincoln County Behavioral Health division is looking for a Community Mental Health Nurse to provide community-based mental health nursing services, including direct patient care and nursing case management. The role involves promoting physical and emotional health, addressing patient health concerns, educating clients on health issues and health choices, and maintaining records in accordance with county policies and procedures. The position also involves maintaining client contact to comply with division productivity standards.
Compensation$40.19 - $54.02 per hour*

Community Health Nurses will follow the six standards of practice:
- Assessment – Conduct home visits and/or office visits, interview clients, review charts, develop a plan of treatment, document assessments in clear language, perform emergency mental health assessment and crisis intervention as required, and observe medication administration when applicable.
- Diagnosis and Priorities – Analyze assessments to determine diagnoses and service priorities; document diagnoses to involve the client in the care plan.
- Outcomes Identification – Identify culturally relevant, health-literate, evidence-based outcomes achievable through local resources; document expected outcomes and measurable objectives.
- Planning – Develop client-centered plans reflecting best practices; document plans using health-literate language understood by the care team and the individual served.
- Implementation – Oversee plan implementation through treatment, case management, referrals, administration of long-acting injectables, collaboration with community resources, health promotion and education; document care coordination activities.
- Evaluation – Continuously assess progress and revise plans as needed; document changes to improve intervention effectiveness.
- Attend and participate in team meetings, trainings, and activities to support community mental health.
- Participate as a member of the interdisciplinary treatment team for identified clients as indicated.
- Provide staff training relative to best practices, as assigned.
- Associate’s degree in nursing from an accredited school of nursing.
- Two years of experience in a mental health setting (lived experience with mental health may substitute for years of experience).
- Must possess a valid Oregon Driver’s License within 30 days of hire.
- Must possess a valid license to practice in the State of Oregon as a Registered Nurse, current and active at time of hire and renewals.
- Bachelor’s degree in nursing preferred.
- Bilingual and/or bicultural competencies for the Latino population preferred.
- Principles, practices, methods, and procedures of nursing in community and public mental health.
- Uses and effects of medications; issues of alcohol and drug abuse.
- Strength-based model of behavior change; problem-solving and motivational interviewing.
- Utilization of community resources; medical record keeping; knowledge of private and public agencies.
- Build therapeutic relationships and maintain boundaries; engage clients in mutual goal setting.
- Provide professional nursing care independently; identify health problems and plan/implement solutions.
- Administer medications; interpret and apply policies and guidelines; maintain records; adapt to changing situations.
- Use data and evidence-based practices to inform nursing practice; maintain HIPAA and county confidentiality.
