Job Overview
We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. This role involves providing comprehensive patient care across various settings, including home health and community care. The ideal candidate will have a strong background in physiotherapy, anatomy, physiology, and medical terminology, with experience in diverse patient populations such as pediatrics, geriatrics, orthopedics, and acute care. The Physical Therapist will develop and implement individualized care plans, coordinate discharge planning, and utilize EMR systems to ensure seamless patient management.

- Assess patients’ physical conditions through detailed evaluations and develop personalized treatment plans.
- Implement physiotherapy techniques including trigger point therapy, orthopedics interventions, and rehabilitation exercises.
- Manage discharge planning to ensure smooth transition of care from hospital or clinic to home or community settings.
- Provide patient education on injury prevention, mobility enhancement, and health maintenance strategies.
- Document patient progress accurately using EMR systems and communicate effectively with healthcare teams.
- Collaborate with physicians and other healthcare professionals to optimize treatment outcomes.
- Monitor patient responses to therapy and adjust treatment plans as necessary for optimal recovery.
- Assist in managing acute care cases and provide specialized care for pediatric, geriatric, or outpatient populations as needed.
- Strong knowledge of anatomy, physiology, and medical terminology relevant to physiotherapy practice.
- Experience in discharge planning, inpatient and outpatient care, hospital settings, home health, and community care environments.
- Proficiency with EMR systems for documentation and patient record management.
- Skilled in various physiotherapy techniques including trigger point therapy and orthopedic rehabilitation.
- Excellent patient care skills with the ability to communicate effectively across diverse populations including pediatrics and geriatrics.
- Ability to work independently as well as part of a multidisciplinary healthcare team.
- Familiarity with care plans development for different clinical settings such as acute care, outpatient clinics, home & community care. Pay: $55.00 - $65.00 per hour
