We are seeking a Registered Nurse to provide palliative and end‑of‑life care in our Newport News, Virginia facility. The position offers an hourly rate of $33.00 to $66.00, determined by program, work commitment, and business needs.
Responsibilities- Assess, plan, implement, and evaluate the total nursing care of assigned patients.
- Develop initial and ongoing comprehensive assessment of client needs, including physical, functional, psychological, and environmental aspects.
- Assess patient risk for pathological grief, cultural and spiritual implications, and communication patterns.
- Evaluate caregiver’s ability to meet patient needs upon admission and throughout care.
- Determine scope and frequency of services based on acuity and patient/family needs.
- Complete clinical documentation in accordance with Riverside Health System standards, including patient notes, verbal orders, certifications, and recertifications.
- Apply admission and re‑certification criteria for hospice care to establish appropriate levels of care and eligibility.
- Implement individualized plan of care, recommending revisions as necessary.
- Manage patient discomfort and provide symptom relief.
- Provide specialized nursing skills related to palliative and end‑of‑life care.
- Consult, counsel, and educate patients and families on nursing needs, disease process, self‑care techniques, end‑of‑life care, and ethical issues.
- Use a case‑management approach to refer patients to other services and coordinate patient/family services with interdisciplinary teams.
- Provide ongoing evaluation of patient/family response to care and communicate changes to the physician and IDT.
- Initiate preventive and rehabilitative nursing procedures to support physiological and psychosocial outcomes.
- Prepare clinical and progress notes demonstrating progress toward goals.
- Supervise LPNs/LVNs and home health aides, ensuring compliance with regulatory guidelines.
- Provide specialized hospice training to staff, family members, and informal caregivers.
- Participate in training and support of all team members, volunteers, and students.
- Delegate and prioritize tasks to ensure quality patient care.
- Recognize and respond to emergency situations.
- Evaluate personal support needs and utilize available resources.
- Registered Nurse license (required).
- Bachelor’s degree in Nursing (preferred).
- Hospice experience (preferred).
- Registered Nurse – Virginia Department of Health Professions (required).
- CPR/BLS Certification from the American Heart Association, American Red Cross, or American Safety and Health Institute within 30 days of hire (required).
