LVN ​/ LPN – LTC​/SNF

LVN / LPN Contract Position

We are seeking a dedicated LVN / LPN for a contract position within Long-Term Care / Skilled Nursing Facilities. The role involves providing quality nursing care to residents in a fast-paced environment.

Responsibilities:

• Administer medications and treatments accurately according to care plans.
• Monitor and document patient vital signs and health status.
• Collaborate with interdisciplinary team to develop and update patient care plans.
• Ensure compliance with facility policies and regulatory standards.
• Assist residents with activities of daily living and provide compassionate support.
• Maintain accurate and complete documentation of patient care activities.

Qualifications:

• Valid LVN or LPN license in the state.
• Proven experience working in LTC or SNF settings.
• Ability to adhere to strict safety and infection control protocols.
• Strong communication and interpersonal skills.
• Must possess current CPR and First Aid certifications.
• Ability to work independently and as part of a team.
