LVN ​/ LPN – LTC​/SNF

LTC/SNF Nurse

Provide skilled nursing care to residents in long-term care or skilled nursing facilities, ensuring their health, safety, and comfort. Collaborate with interdisciplinary teams to deliver high-quality patient care.

Responsibilities:

• Administer medication and treatments as prescribed by physicians.
• Monitor and document residents' health status and progress.
• Assist with activities of daily living, including bathing, dressing, and mobility.
• Ensure compliance with facility policies, procedures, and safety standards.
• Communicate effectively with residents, families, and healthcare team members.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Current CPR and BLS certification.
• Minimum of 1 year of experience in LTC or SNF settings.
• Ability to work every other weekend and adapt to a flexible schedule.
• Must bring valid government-issued  wear a photo badge at all times.
• Willingness to comply with vaccination policies or provide exemptions as needed.
