Position: Registered Nurse (RN) Newport Location

Job Summary

The Registered Nurse (RN) provides skilled nursing care to patients in their homes, focusing on promoting health, preventing complications, and delivering compassionate end-of-life care when appropriate. This role involves assessing patient needs, developing care plans, coordinating with interdisciplinary teams, and supporting patients and families.

Key Responsibilities
  • Perform comprehensive patient assessments in the home setting.
  • Develop, implement, and update individualized care plans.
  • Administer medications, treatments, and wound care as prescribed.
  • Monitor patient condition and report changes to physicians or care team.
  • Provide education to patients and families on disease management and care.
  • Deliver compassionate hospice care focused on comfort and quality of life.
  • Coordinate care with physicians, therapists, social workers, and aides.
  • Maintain accurate and timely documentation in compliance with regulations.
  • Ensure adherence to infection control and safety standards.
  • Participate in on-call rotation as required.
Qualifications
  • Current Registered Nurse (RN) license (state‑specific).
  • Graduate of an accredited nursing program.
  • Minimum 1–2 years of clinical nursing experience (home health or hospice preferred).
  • Valid driver’s license and reliable transportation.
  • Current CPR certification (BLS required; ACLS preferred).
Skills & Competencies
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work independently and manage time effectively.
  • Compassionate, patient‑centered approach to care.
  • Knowledge of home health and hospice regulations (Medicare/Medicaid).
Work Environment

Patient homes and community settings; frequent travel required within service area; flexible schedule, including evenings, weekends, and on‑call shifts.

Physical Requirements
  • Ability to lift/move patients (with assistance as needed).
  • Prolonged standing, walking, and driving.
  • Ability to perform hands‑on patient care tasks.
