Primary Care Registered Nurse (RN)
The Primary Care RN is responsible for managing daily provider schedules while ensuring patients immediate care needs are met in collaboration with other members of the health care team. The RN performs telephone triage and develops a plan of care based on the assessment and per scope of practice. The RN works closely with the primary care provider, relaying messages on patient symptoms and providing feedback to the patient on the updated plan of care.
Key Responsibilities:
- Performs telephone triage based on clinic standing orders and RN scope of practice.
- Relays messages and results to patients and assists in developing a plan of care.
- Orders medications, tests, and referrals and ensures patient understanding.
- Refills or adjusts medications per office policy and standing orders while ensuring patient compliance and safety.
- Works with all members of the healthcare team and refers to other members as patients needs arise.
- Directs LPNs, MAs, and LNAs with daily duties.
Knowledge, Skills and Abilities:
- Maintain a current VT or multistate RN licensure.
- Maintain a current BLS.
- Must possess the ability to multitask and works well under pressure of constantly changing priorities throughout the day.
- Previous experience working in a Primary Care office a plus.
- Previous experience working with pediatric patients a plus.
Pay Information: RN: $43.49 hourly
Primary Care Licensed Practical Nurse (LPN)The Primary Care LPN is responsible for managing providers daily schedules while ensuring patients immediate care needs are met, collaborating with other members of the health care team as needed. The LPN provides patient education on diagnosis, treatment options, and medications, including potential side effects and manage medication dosing/changes based on clinic standing orders.
Key Responsibilities:
- Relays messages and results to patients and assists in developing a plan of care.
- Orders medications, tests, and referrals and ensures patient understanding.
- Gives immunizations based on standing orders and CDC guidelines.
- Refills or adjusts medications per office policy and standing orders while ensuring patient compliance and safety.
- Works with all members of the healthcare team and refers to other members as patients needs arise.
- Directs MAs and LNAs with daily duties.
Knowledge, Skills and Abilities:
- Maintain a current VT or multistate LPN license.
- BLS required.
- Must possess the ability to multitask and works well under pressure of constantly changing priorities throughout the day.
- Previous Primary Care experience a plus.
- Previous pediatric experience a plus.
Pay Information: LPN: $29.71 hourly
