LVN​/LPN – LTC​/SNF

Job Summary:

Provides nursing care to residents in long-term care or skilled nursing facilities as an LVN/LPN. Ensures delivery of quality, compassionate patient care under the supervision of registered nurses and healthcare team members.

Responsibilities:

• Administer approved medications and treatments in accordance with facility policies.
• Monitor residents’ health status and document vital signs, symptoms, and responses to treatment.
• Communicate effectively with residents, families, and healthcare team members regarding patient care needs.
• Assist with daily living activities to promote resident comfort and well-being.
• Follow all infection control procedures, including COVID-19 protocols, and wear proper identification at all times.
• Maintain accurate, timely documentation of resident care activities and observations.

Qualifications:

• Valid and current LVN/LPN license in the state.
• Proven experience in long-term care or skilled nursing facilities.
• Knowledge of clinical procedures, medication administration, and documentation standards.
• Ability to work effectively in a team and adhere to facility protocols.
• Strong communication and interpersonal skills.
• Successful completion of all required COVID-19 vaccination or approved exemptions.
