LVN​/LPN – LTC​/SNF

Job Summary:

Provide high-quality nursing care to patients in a long-term care or skilled nursing facility setting. Ensure patient safety, comfort, and well-being through supervised clinical responsibilities and effective communication.

Responsibilities:

• Administer medications and treatments following current care plans.
• Monitor patient vital signs and assess physical and emotional needs.
• Document patient conditions, interventions, and responses accurately.
• Collaborate with interdisciplinary team to develop and update care strategies.
• Ensure compliance with facility policies, state, and federal regulations.
• Maintain a safe, clean, and supportive environment for residents.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience working in LTC/SNF settings preferred.
• Ability to follow detailed care plans and document accurately.
• Strong communication and teamwork skills.
• Knowledge of medication administration, infection control, and patient safety standards.
• Must bring a state or federal issued  wear a photo badge during shifts.
