• Administer medications and treatments per physician orders and facility policies
• Monitor assigned residents, assess for changes, document findings, and report to the supervising nurse
• Perform routine nursing tasks, wound care, dressing changes, and basic procedures as directed
• Collaborate with the registered nurse, care team, and interdisciplinary staff to update care plans
  
  Maintain patient safety, infection control, and accurate charting in the facility record
• Respond promptly to call bells, prioritize tasks, and provide compassionate bedside care during night hours

• Active LPN license in Vermont (or ability to obtain as required for this assignment)
• Ability to work 3x12 night shifts (18:45–07:15)
• Demonstrated ability to provide patient-centered care in a long‑term care setting
• Strong communication and teamwork skills
• Eligible for travel/temporary assignments and able to meet facility health and safety requirements
