We are seeking a Home Health RN/Case Manager who is passionate and knowledgeable as well as has good time management skills. Home health offers flexibility in your work week. As a RN Case manager in home care, you'll have the privilege of providing skilled nursing care that is customized to the patient's home and daily life. • Graduate of an accredited nursing program with ana active RN license

• Minimum of 1 year of geriatric experience preferred

• Strong Organizational and self-management skills
• Clinical proficiency

• Basic Cardiac Life Support, RN License, Driver’s license and Vehicle with Current Auto Insurance
