TheOpportunity
In this full-time, on-site role based in Newton, Massachusetts, the Case Manager is a key member of Asteroid Health’s interdisciplinary treatment team. The Case Manager provides direct care coordination, case management, and client advocacy across Asteroid Health’s continuum of mental health services. This position works closely with clinicians, prescribers, and program leadership to support treatment planning, facilitate referrals, coordinate services, and ensure continuity of care.
The Case Manager plays an essential role in supporting client engagement, monitoring progress toward treatment goals, and maintaining compliance with clinical documentation and regulatory standards.
- Provide comprehensive case management services to clients across Psychiatric Day Treatment, Intensive Outpatient Programming (IOP), and outpatient levels of care.
- Coordinate care among clients, families, clinicians, prescribers, and external providers to ensure continuity and quality of treatment.
- Participate in interdisciplinary treatment team meetings and contribute to individualized treatment planning.
- Assist clients with referrals and linkage to community resources, including primary care, psychiatry, housing, vocational services, and social supports.
- Support client engagement, retention, and successful transitions between levels of care and at discharge.
- Monitor client progress toward treatment goals and communicate relevant updates to the treatment team.
- Conduct care coordination activities such as appointment scheduling, follow-up outreach, and collateral communication (with appropriate releases).
- Maintain timely, accurate, and compliant clinical documentation in accordance with agency policies, state regulations, and payer requirements.
- Support discharge planning, including aftercare coordination and relapse prevention planning.
- Advocate for clients’ clinical, social, and practical needs while promoting recovery-oriented, trauma-informed care.
- Adhere to all confidentiality, ethical, and professional standards, including HIPAA and Massachusetts regulatory requirements.
- Collaborate with program leadership to support quality improvement initiatives and program operations as needed.
- Bachelor’s degree in Social Work, Psychology, Human Services, or a related field (Master’s preferred).
- Experience in case management or behavioral health settings preferred.
- Knowledge of mental health diagnoses and recovery-oriented care.
- Strong organizational, communication, and documentation skills.
- Ability to manage a caseload and work collaboratively on an interdisciplinary team.
- Familiarity with Massachusetts community resources and referral processes preferred.
- Proficiency with electronic health records and basic office technology.
