Role: Travel Home Health Physical Therapist
Responsibilities: Provide physical therapy services to patients in their homes across multiple locations within the North Region, treating individuals of all ages with physical impairments or disabilities. Collaborate with patients, families, physicians, and healthcare professionals to assess needs and coordinate care. Maintain thorough electronic documentation and ensure timely communication according to agency standards. Cover various locations as assigned, using Epic for charting.
Qualifications & Skills: Massachusetts Physical Therapist license (if applicable), minimum of 1 year home care experience, at least 1 year of travel assignment experience, and AHA certifications. Must have a valid flu vaccination with no exemptions. Ability to travel to different cases and work independently in a home health setting.
Key Duties: Deliver quality physical therapy in patients' homes, document care accurately, and work collaboratively with interdisciplinary teams. Maintain compliance with policies and standards. Provide care to a diverse patient population, from birth to over 100 years old.
Additional Info: Local candidates within 60 miles of Boston may receive a reduced rate. RTO must be specified upfront. The schedule involves day shifts, 8 hours per day, 40 hours weekly. The assignment spans 13 weeks, focusing on providing patient-centered care in a home environment.
