Full-Time RN Case Manager

Temp-to-Perm or Direct-Placement

Location: In-home assessments between Framingham and Canton, MA
Schedule: Monday-Friday | 9:00 AM-5:00 PM (flexible start/end time) | 40 hours/week


Compensation

Pay Rate: $57/hour
Direct Hire Salary: $100,000/year
Mileage Reimbursement: $0.45/mile


Position Overview

Amergis Healthcare Staffing is partnering with a respected Home Health organization dedicated to helping older adults and individuals with disabilities remain safe, independent, and supported in their homes. Through individualized care plans, clinical oversight, patient and family education, and comprehensive case management, this organization delivers high-quality, community-focused care.

As an RN Case Manager, you will take a lead role in clinical assessments, ongoing evaluations, and the development of individualized care plans. You will ensure continuity of care, maintain accurate documentation, and collaborate closely with physicians and community health partners to implement proactive, patient-centered strategies.


Key Responsibilities
  • Complete initial patient assessments and full physical evaluations in the home.
  • 4-5 visits per day
  • Conduct ongoing re-evaluations as patient needs change.
  • Develop, implement, and revise individualized care plans.
  • Utilize assessment data to establish nursing diagnoses and set care goals.
  • Implement preventive and rehabilitative nursing procedures.
  • Administer medications and treatments per physician orders.
  • Provide patient and family counseling and health education.
  • Prepare accurate clinical notes and update physicians regularly (minimum every 60 days).
  • Communicate changes in condition and obtain physician orders as needed.
  • Coordinate care with physicians, caregivers, and community health resources.
  • Identify concerns related to safety, psychosocial issues, or medical status changes.
  • Participate in interdisciplinary communication and team-based care planning.
  • Coordinate necessary equipment and services for patients.
  • Instruct, supervise, and evaluate home health aides every two weeks.
  • Perform additional duties as assigned.
Qualifications
  • Active Massachusetts RN license in good standing.
  • Graduate of an accredited nursing program.
  • 1-2 years of recent acute care experience required.
  • Home health experience preferred.
  • Strong clinical assessment and autonomous decision-making skills.
  • Excellent communication, documentation, and problem-solving abilities.
  • Ability to perform physical tasks such as lifting, positioning, and transferring patients.
  • Valid driver's license and reliable, insured vehicle meeting organizational requirements.
  • Experience with EMRs and clinical documentation systems.
  • CPR certification required.
  • Supervisory experience not required; role includes oversight of home health aides.
