Registered Nurse (RN) Case Manager - Full Time

Service Area: Home visits conducted throughout the region between Framingham and Canton, MA
Schedule: Monday-Friday | 9:00 a.m.

-5:00 p.m. (40 hours/week)
Employment Type: Temp-to-Perm or Direct Hire

Compensation
  • Contract Rate: $57/hour
  • Permanent Salary: $100,000 annually
  • Mileage Reimbursement: $0.45 per mile
Role Summary

Amergis Healthcare Staffing is collaborating with a well-established Home Health provider dedicated to helping older adults and individuals with disabilities maintain independence, safety, and overall well-being in their own homes.

As an RN Case Manager, you will be responsible for delivering patient-centered, community-based nursing care. This includes completing assessments, developing personalized care plans, overseeing clinical care, providing education, and managing all aspects of case coordination for your assigned patients.

The Registered Nurse organizes and delivers in-home clinical services, leveraging community resources to support patients and families effectively.


Key Responsibilities

Clinical Care
  • Perform comprehensive initial assessments and complete full physical evaluations.
  • Conduct regular follow-up assessments to re-evaluate patient needs.
  • Develop, implement, and adjust care plans based on patient conditions and goals.
  • Use assessment findings to determine nursing diagnoses.
  • Collaborate with patients and families to establish meaningful care goals.
  • Carry out preventive and rehabilitative nursing interventions.
  • Administer treatments and medications as ordered by the provider.
  • Provide counseling and education to patients and families regarding health needs.
  • Deliver health instruction aligned with the established care plan.
  • Support and manage discharge planning processes.
  • Serve as the designated Case Manager for assigned patients under the direction of the Clinical Supervisor.
Communication & Coordination
  • Document accurate, timely clinical notes and update the referring physician at least every 60 days.
  • Notify the physician of any significant change in condition and obtain needed orders.
  • Coordinate services with physicians, families, and community health resources.
Additional Duties
  • Participate in the on-call rotation as required by agency policy.
  • Arrange for necessary equipment and supportive services for patients.
  • Provide instruction, oversight, and bi-weekly evaluations for home health aides.
Qualifications
  • Graduate of an accredited school of nursing.
  • 1-2 years of recent acute care experience in a hospital or similar setting.
  • Current RN license in good standing and valid CPR certification.
  • Bachelor's degree preferred; home health experience (1+ year) preferred.
  • Strong clinical judgment, communication, problem-solving, and documentation abilities.
  • Ability to perform physical tasks such as lifting, positioning, and transferring patients.
  • Valid driver's license and reliable, insured vehicle that meets organizational/state standards.
  • Prior supervisory experience not required; this role includes oversight of home health aides.
