Join a team that is redefining how acute care is delivered. As part of the Mass General Brigham Home Hospital program, you'll provide hospital-level care directly in the patient's home—offering comfort, dignity, and personalized attention while helping reduce hospital stays and improve outcomes.
This role offers the best of both worlds: the autonomy of community-based nursing and the clinical challenge of caring for patients with acute, inpatient-level needs. You'll work with a supportive interdisciplinary team and participate in innovative care models designed to improve access, quality, and patient experience.
Per Diem RN Team members are asked to commit to a minimum of 4 shifts per month (8:00 AM - 8:30 PM), with two of those falling on a weekend day.
This position supports patients across the South/West Region, including Boston and its residential areas, as well as Framingham, Natick, Newton, Dedham, Quincy, Sharon, and nearby communities.
Who You'll Care For:- Acute medical conditions such as heart failure, COPD exacerbations, pneumonia, cellulitis, or dehydration
- Post-ED or post-hospitalization needs requiring short-term, skilled monitoring
- Infections requiring IV therapies, including antibiotics and fluid management
- Chronic disease exacerbations needing rapid assessment and stabilization
- Diagnostic follow-up, remote monitoring, and daily clinical oversight
We typically care for up to five patients a day. We start and end our day with virtual team rounds so we can stay connected and aligned. Throughout the day, we communicate regularly to support patient needs, coordinate care, and ensure continuity.
Job SummaryProvide high-impact, patient-centered care by completing thorough nursing assessments and creating individualized care plans, delivering advanced, skilled nursing interventions—including treatments, education, and inpatient-level clinical support—in the patient's home, monitoring patient status using remote physiologic monitoring and responding promptly to changes in condition, coordinating comprehensive care—including diagnostics, medication reconciliation, durable medical equipment (DME), and interdisciplinary services, documenting care efficiently and accurately using Point-of-Care electronic records, providing patient and caregiver teaching, supporting safe transitions, and assisting with discharge planning, and managing your daily schedule with autonomy aligned with patient needs and program priorities.

- Document care efficiently and accurately using Point-of-Care electronic records.
- Provide patient and caregiver teaching, support safe transitions, and assist with discharge planning.
- Manage your daily schedule with autonomy, aligned with patient needs and program priorities.
- Graduate of an approved, accredited school of professional nursing required; BSN preferred
- Current license to practice as a Registered Nurse in the Commonwealth of Massachusetts required
