Licensed Practical Nurse

Job Summary

Mass General Brigham Home Care seeks a Licensed Practical Nurse (LPN) to provide 1:1 skilled nursing care in the community. The role includes delivering complex clinical treatments, collaborating with RN case managers, and practicing at the top of the license. LPNs will focus on preventing readmissions and improving patient lives.

• Deliver 1:1 skilled nursing care.
• Work collaboratively with RN case managers.
• Practice at the top of the license.
• Provide complex clinical treatments.
• Support patients recovering from surgery, illness or chronic disease exacerbations.

• Full-time (40 hrs) preferred; 32‑hr and 36‑hr options available.
• Monday‑Friday schedule.
• Average 6–7 visits per day.
• Dedicated time for documentation, care coordination, and chart review.

• LPN license in Massachusetts.
• Minimum 1‑year previous nursing in acute or sub‑acute care.
• Home‑care experience preferred.
• Effective written, verbal and electronic communication skills.
• Ability to use computer software; hand‑held device knowledge preferred.
• Knowledge and skills to care for geriatric, pediatric and/or adult populations.
• Strong interpersonal and organizational skills; ability to work independently and adapt to unpredictable circumstances.
• Travel within local geographic area required.

Hybrid. 75‑95 Wells Ave. 40 scheduled weekly hours. Regular employee type. Day shift (United States).

$29.13 – $43.64 per hour.
