Pay: $2,183 – $2,509 per week

Shift: Days – 5 days x 8 hours

Contract Duration: 13 weeks

A Home Health Registered Nurse (RN) delivers direct nursing care to patients in their homes, ensuring comfort, stability, and effective management of medical conditions. The RN assesses patient conditions, develops individualized care plans, administers treatments, and educates patients and caregivers on disease management.

• Assess patient conditions and develop individualized care plans based on medical needs and physician directives
• Administer medications, treatments, and wound care while monitoring patient responses
• Provide patient and caregiver education on medication management, disease progression, and self‑care techniques
• Monitor for changes in patient status, reporting concerns and adjusting care plans as needed
• Coordinate with physicians, therapists, and other healthcare professionals to ensure seamless care transitions
• Document patient care, interventions, and progress accurately and in compliance with regulatory standards
• Ensure compliance with infection control, safety, and home care best practices to protect patients and caregivers
• Support families and caregivers, offering guidance and emotional support throughout the care process
• Maintain communication with case managers and interdisciplinary teams to provide coordinated patient care
• Complete charting and documentation requirements while meeting visit and productivity goals
• Perform other duties as assigned within the scope of practice

• Critical thinking, service excellence, and good interpersonal communication skills
• Ability to read, write, and communicate in English
• Proficiency in PC skills and following written and verbal instructions
• Physical abilities: can remain stationary, move equipment (50–100 lb), bend, push, pull
• Interpersonal skills: works effectively with a variety of personnel
• Technical skills: manipulation and use of equipment for procedures
• Mental requirements: cope with frequent public contact, meet deadlines under pressure, handle irregular schedules
• Sensory: visual acuity and effective communication

• One year of recent experience as a Registered Nurse, including a minimum of six months in a Home Health setting
• Proficient in OASIS documentation
• Valid RN license in compliance with state regulations
• BLS certification (AHA/ARC)

• Home Health Nursing Certification (RN‑BC)
• Other certifications and licenses as required for the position
