Job Summary
  • Provide 1‑on‑1 nursing care to patients in their homes, schools, or community settings, helping families navigate complex medical care and stay at home.
  • Deliver continuous care nursing for medically complex children and adults.
  • Manage a diverse caseload including feeding challenges, G or J‑tube feeding, tracheostomy/ventilator support, seizure disorders, ostomy appliances, wound care, and diabetic maintenance.
  • Work collaboratively with interdisciplinary teams to ensure high‑quality, patient‑centered care.
Pay and Benefits

LPN hourly rates: $40 / $42 / $44 depending on differential.

Qualifications
  • Valid LPN license in the state of Massachusetts.
  • Proficiency in nursing skills, critical thinking, and problem‑solving; experience in diverse medical conditions desirable but new graduates acceptable if supported.

  • Adaptable to varying patient needs, environments, and challenges.
  • Team player who collaborates effectively with healthcare professionals, families, and interdisciplinary teams.
