Position Overview:We are seeking a dedicated and compassionate Advanced Practice Registered Nurse (APRN) to join our multidisciplinary team. As an integral member of the team, you will deliver primary and preventive care services to patients of all ages, with a focus on addressing healthcare disparities and promoting health equity. Key Responsibilities:- Conduct thorough patient assessments, including medical history reviews, physical examinations, and psychosocial assessments.
- Diagnose and treat common acute illnesses and chronic conditions within the scope of practice.
- Order and interpret diagnostic tests, such as laboratory tests and imaging studies.
- Develop and implement evidence-based treatment plans and preventive care strategies.
- Provide patient education on disease prevention, health promotion, and management of chronic conditions.
- Collaborate with physicians, nurses, community health workers, and other healthcare professionals to coordinate patient care and referrals.
- Maintain accurate and up-to-date electronic health records (EHR) and documentation of patient encounters.
- Participate in quality improvement initiatives and clinical committees to enhance patient outcomes and healthcare delivery.
- Master's degree in Nursing from an accredited program.
- Current Advanced Practice Registered Nurse (APRN) license in Massachusetts.
- Certification as a Nurse Practitioner (NP) in Family, Adult-Gerontology, or another relevant specialty.
- Minimum of 2 years of clinical experience as an Advanced Nurse Practitioner preferred, preferably in a community health or FQHC setting.
- Strong commitment to serving underserved populations and addressing healthcare disparities.
- Excellent interpersonal and communication skills, with the ability to work effectively within a culturally diverse patient population.
- Proficiency in electronic health records (EHR) systems and other healthcare technology.
