RN Integrated Care Manager – Community Health Navigator

Care Source Massachusetts seeks an Integrated Care Clinical Manager (RN) to monitor and coordinate care for dually-eligible enrollees with complex needs. You will manage assessments, care planning, and outreach across medical, behavioral, and social services in community settings.

You will lead the ICT, partner with providers, and educate members on Medicare/Medicaid benefits while promoting preventive care and self-management. Significant travel within MA is required.
