- Identify residents’ psychosocial, mental, and emotional needs
- Complete social histories and psychosocial assessments
- Collaborate with the interdisciplinary team to protect and promote residents’ rights
- Prevent and address resident abuse per law and professional standards
- LTC/Subacute experience required
- LSW and/or CSW required
