Licensed Practical Nurse (LPN) – Provide compassionate care to residents.

• Provide direct patient care, administer medications, monitor vital signs, and perform routine tasks.
• Assess patients' conditions, identify areas of concern, and report to the healthcare team.
• Develop and implement individualized care plans in collaboration with interdisciplinary team.
• Monitor and maintain accurate medical records, ensuring timeliness and accuracy.
• Maintain a clean and organized environment, adhering to infection control protocols.
• Collaborate with other healthcare professionals to ensure seamless care transitions.
• Participate in quality improvement initiatives.

• Licensed Practical Nurse (LPN) license in New Jersey.
• Strong communication, interpersonal, and teamwork skills.
• Ability to work effectively in a fast-paced, dynamic environment.
• Compassionate and empathetic approach to patient care.
• Basic computer skills, including electronic medical records.
