Job Summary:

We are seeking a Registered Nurse to join our dedicated team of healthcare professionals at Mohawk Meadows Rehab & Nursing Center. As a Registered Nurse, you will play a vital role in providing high-quality, patient-centered care to our residents. Your expertise and compassion will enable us to deliver exceptional services that promote healing, comfort, and dignity.

Responsibilities:
  • Care Delivery: Provide direct patient care, including administering medications, performing treatments, and providing wound care, as ordered by the physician.
  • Assessment and Intervention: Conduct initial and ongoing assessments of residents' needs, developing and implementing individualized care plans to address identified needs.
  • Communication: Collaborate with interdisciplinary teams, including physicians, therapists, and other healthcare professionals, to ensure seamless care transitions and effective patient outcomes.
  • Documentation: Maintain accurate and timely records of resident care, progress, and any concerns.
  • Quality Improvement: Participate in quality improvement initiatives to enhance patient care and satisfaction.
  • Support: Assist with orientation and training of new staff members, supporting the growth and development of our team.
Requirements:
  • Certifications: Current Registered Nurse licensure in the state of New Jersey
  • Skills: Excellent communication and interpersonal skills; ability to work effectively in a fast-paced environment; strong attention to detail and ability to prioritize tasks.
