Description

Compensation: $33.00 per hour.

Schedule:

M-F 830a-430p, ocassional weekend.

Benefits:

  • Evaluates patients referred to for Hospice and Palliative Care to determine appropriateness for admission. Routinely accepts all referrals for Hospice and Palliative Care; does not refuse any patient without prior approval of administration.
  • Consults with physicians, team members, patients and/or family members while establishing a coordinated referral.
  • Evaluates insurance coverage and provides this information to persons involved with billing. Coordinates efforts with Social Services.
  • Notifies the team of patient referrals and provides patient data including essential background information and initial medical orders. Obtains physician verbal orders where needed.
  • Provides feedback to referral sources on referrals as indicated.
  • Responds to telephone inquiries regarding available services and eligibility for same.
  • Collects and maintains statistical data on all referred patients and submits same, as requested, to administration.
  • Registers all patients in the EMR, and responsible for new admission chart generation.
  • Performs ICD-10 coding and patient registration.
  • Collaboratively sets QI agenda for Hospice annually; gathers/tabulates data; presents findings; makes recommendations for change. Participates in committees as assigned.
  • Covers on-call on a rotational basis.
  • Triages patient/family calls from patients interested in hospice and/or palliative care services
  • Triage and schedule Quinlan Palliative Care patients on service and communicate to Nurse Case Manager or NP.
  • Submit weekly report to Nursing Supervisor and Executive Director of Referrals, Admissions and Pending
  • Adheres to employment policies as described in the employee handbook; maintains and respects confidentiality; adheres to all HIPAA regulations.
  • Participates in all required in service education programs and meetings.
  • Demonstrates compliance with the dress code policy by appearing well groomed, wearing  while on duty and maintaining a professional appearance.
  • Performs other duties as assigned.
Position Requirements Required Skills/Abilities
  • Knowledge of the dying process; knowledge of policies and procedures of the hospice organization.
  • Knowledge of palliative care principles, practices, and patient-centered care approaches.
  • Knowledge of regulations surrounding compliance with Medicare Conditions of Participation.
  • Ability to function as a resource to patients and families during times of high stress and volatility.
  • Ability to assess patients to determine appropriateness of participation in hospice and Palliative care.
  • Ability to build constructive relationships with colleagues, and outside agencies, organizations, and individuals.
  • Excellent communication skills to effectively deliver information between referral source and the clinical team.
  • Consistently model respect for, and accountability to, the public, patients/families, and co-workers. Communication and actions reflect care, courtesy, honesty, and professionalism.
  • Convey a professional and positive image and attitude regarding the organization.
Education and Experience

Required

Education:

Graduate of an NLN accredited school of nursing; current LPN or RN licensure in New Jersey

Preferred Education: BSN;
Pennsylvania Nursing License

Required Experience: Three (3) years of experience in a Medicare-certified Hospice/Home Care agency under qualified nursing supervision. Evidence of leadership ability; including evidence of ability to take initiative, delegate authority appropriately, ability to role model, ability to serve as a change agent.

Preferred Experience: Experience in Palliative Care.
