Every Age in Newton, NC is seeking an experienced RN to coordinate MDS assessments and resident care plans for a geriatric/long-term care population. You will work with an interdisciplinary team to ensure timely, accurate documentation and compliance with MDS regulations.
In this full-time role you will lead weekly care planning meetings, monitor status changes, assist with admissions and discharges, analyze quality data, and contribute to quality improvement initiatives while educating staff.
