St. Mary Home Care and Hospice is seeking a full time Hospice Registered Nurse-RN Case Manager. St. Mary Home Care and Hospice located in Bucks County, PA, provides compassionate, exceptional care where people are most comfortable: are the area most comprehensive home care provider with trusted quality of care. We have a pioneering care model with Home Care Connect, our integrated virtual care program that helps patients avoid preventable ER visits and hospitalizations. It enhances our clinical excellence with advanced, easy-to-use remote monitoring technology and 24/7 access to our Virtual Care Center RNs.
Responsible for case managing a team of hospice patients. The RN case manager will collaborate with the interdisciplinary team and attend weekly Interdisciplinary meetings. The RN case manager will interact with the primary care physician and/or hospice medical director as needed to effectively manage patient symptoms.
Minimum Qualifications- Graduate of an approved nursing education program
- Licensure as a Registered Nurse in the State of Pennsylvania
- 2 years experience as a professional nurse in acute care, home care or ER experience required
- Home care or Hospice experience preferred but not required
- EPIC experience preferred
