Hospice RN Case Manager

Job Summary

St. Mary's Home Care & Hospice is seeking a full‑time Hospice RN Case Manager. The RN will provide professional nursing care to hospice clients as prescribed by the physician and will be responsible for client assessment, care planning, therapeutic intervention, and overall supervision of client care and outcomes. The RN is the case manager for hospice patients, collaborates with the interdisciplinary team, attends weekly interdisciplinary meetings, and manages patient symptoms.

• Provide professional nursing care to hospice clients as prescribed by physicians.
• Perform client assessment, develop care plans, and implement therapeutic interventions.
• Act as case manager, overseeing hospice patients' care and outcomes.
• Collaborate with interdisciplinary team and attend weekly interdisciplinary meetings.
• Coordinate with primary care physicians or hospice medical directors to manage patient symptoms.
• Support Hospice Aides and LPNs/LVNs as needed.
• Identify and address family/caregiver grief and loss issues and assist in implementing hospice plans of care.

• Graduate of an approved nursing education program.
• Registered Nurse licensure in the State of Pennsylvania.
• Minimum two years of professional nursing experience in acute care, home care, or ER.
• Home care experience preferred but not required.
• EPIC experience preferred.
