Position:
Home care LPN for St. Mary's Home Care. This role involves one-to-one skilled nursing care in patients' homes, creating individualized care plans, administering medications, educating patients and families, and supporting recovery and independence.

Client assessment, care planning, intervention, and overall supervision of patient care and outcomes.

Deliver skilled nursing care in the comfort of patients’ homes.

Create individualized care plans, administer medications, educate patients/families, and support recovery and independence.

• Graduate of an accredited or state approved school of practical nursing.
• Licensure as an LPN in the State of Pennsylvania.
• A minimum one year of experience in an acute care setting; home care experience preferred.
• Phlebotomy and wound care experience required.
• Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
