Social Worker - Social Care Qualified

Assess the needs, circumstances and risks of adults, children and families requiring social care support.

Develop, implement and review personalised care and support plans in partnership with service users and their families.

Safeguard vulnerable individuals and respond appropriately to concerns relating to abuse, neglect or exploitation.

Carry out statutory social work duties in accordance with relevant legislation, policies and professional standards.

Build effective and professional relationships with service users, carers, families and other professionals.

Coordinate and work collaboratively with multi-agency partners, including health, housing, education and voluntary sector services.

Monitor and review the effectiveness of care packages, interventions and support services.

Maintain accurate, confidential and up-to-date case records, assessments and professional reports.

Advocate for service users and promote their rights, independence, wellbeing and participation in decision-making.

Manage an allocated caseload effectively, prioritising urgent needs and ensuring assessments, reviews and interventions are completed within required timescales.
