Base salary ranges between $100,000 and $120,000 with total earnings potential reaching $130,000+ with our monthly incentive plan based on performance.

• Full‑time or Part‑time opportunities available
• Exciting opportunity to grow your medical skills while providing personalized, high‑quality geriatric day‑to‑day care to seniors and patients with complex needs where they reside
• Practice evidence‑based clinical care by planning, directing, and evaluating patient medical needs then treating chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses
• Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care from post‑hospitalization to home

• Current Nurse Practitioner National Board Certification
• Active and unrestricted APRN license under the State Board of Medicine
• Current DEA, BLS and ACLS certifications
• Master’s degree in nursing (MSN)
• Five or more years of nursing experience with at least two years of NP experience preferred
