The Avalon of Lewis Center is recruiting skilled, compassionate, detailed‑oriented LPNs to join our team! This position provides direct nursing care to residents and delegates and supervises the care provided by our Quality‑of‑Life Specialists/Resident Assistants.

Here are a few of the daily responsibilities of an LPN:

• Provide daily supervision to assigned staff
• Responsible for identification of resident medical and behavioral changes and the development and monitoring of care plans and re‑assessment of each resident.
• Accurately administer prescribed medications, supervise the continuity of nursing care, respond to changes in medical conditions, medications.
• Assess/ monitor and treat medical conditions.
• Participate in admission of new residents
• Support DOHS in completion of assessments and evaluation of residents health and safety.

Here are a few of the qualifications we need you to have

• Current nursing license valid in state worked
• Ability to speak, read and comprehend English language
• Good Communication and teamwork skills

Salary Range - $30.00-$32.00

Shift differential for evenings and weekends!
