Your Role:
  • Resource Coordination: Connect patients and families with community resources and services to support their well-being.

  • Advocacy: Serve as an advocate for patients, ensuring their social, emotional, and financial needs are met.

  • Collaboration: Work closely with healthcare professionals to integrate social services into comprehensive care plans.

  • Program Support: Assist in the development and implementation of social services programs tailored to patient needs.

  • Documentation: Maintain accurate records of patient interactions and service referrals in compliance with regulatory standards.

What You Bring:

Bachelor’s Degree in Social Work, Psychology, or a related field (or equivalent experience).
Experience in Social Services or a similar role, preferably within a healthcare setting.
Strong Communication & Interpersonal Skills to engage effectively with patients and community partners.
Organizational Skills with attention to detail in documentation and service coordination.
