Per Diem Nurse Practitioner

We are seeking a dedicated Per Diem Nurse Practitioner (NP) to join our clinical team. This position provides flexible weekend coverage on an as-needed basis. Hours will vary from week to week, and shifts will be assigned with as little as 12–24 hours’ notice
. This role is ideal for an experienced NP who values flexibility and enjoys working in a dynamic environment. Facilities located in 60714 and 60645 zip codes.

• Provide high-quality patient care in accordance with clinical protocols and organizational standards.
• Perform patient assessments, develop and implement treatment plans, and collaborate with physicians and interdisciplinary team members.
• Document clinical findings and maintain accurate medical records.
• Respond to urgent coverage needs with professionalism and adaptability.

• Minimum of 2 years clinical care experience in geriatrics, long-term care, or internal medicine.
• Familiarity with ISNP models of care and value-based care principles is preferred.
• Proficiency in basic computer skills, virtual meeting platforms, Microsoft and LTC clinical EMRs.
• Excellent communication and documentation skills.
• Good organizational skills with attention to detail

• Positive and enthusiastic attitude.

• Current certification as an Adult, Geriatric or Family Nurse Practitioner in the state of practice.
• Current unrestricted RN license for the state in practice.
• Holds or is eligible for DEA licensure and CDS licensure (if required in the state of practice)
• Meets PPCM credentialing requirements.
• Holds current Basic Life Support certification.
• Holds valid driver’s license and car insurance and has reliable transportation
