We are seeking a Registered Nurse (RN) for Home Health to provide high quality care in patients’ homes.
Primary Coverage Area & ScheduleSt. Joseph and Cass County (MI), with some local coverage outside these counties as needed. Full‑time – Monday‑Friday, 8:00 am‑5:00 pm plus weekend, holiday rotation (no call, no nights!).
Key Responsibilities- Ensure all activities align with Elara Caring’s vision and the leadership of the Home Health team.
- Maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.
- Assess assigned case load and identify physical, psychosocial, and environmental needs of patients and families.
- Plan, coordinate, and implement nursing care plans; evaluate and revise them as needed.
- Conduct initial and re‑evaluations of nursing needs during each visit.
- Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and other team members.
- Make follow‑up visits according to patient, family, and facility needs.
- Current, unrestricted RN license in Michigan.
- At least 1 year of clinical care experience as a nurse.
- Willingness and ability to travel within the coverage area.
- Ability to lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
- Dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
- Preferred:
Associate’s or Bachelor’s degree in Nursing. - Preferred:
Previous Home Health or Hospice experience;
Home Care Home Base experience preferred.
