The Documentation Specialist is responsible for reviewing behavioral health documentation to ensure accuracy, completeness, and compliance with payer, regulatory, and organizational requirements. This position helps ensure that clinical documentation supports services billed, reduces billing errors and denials, and promotes overall documentation quality.

Required qualifications:
  • Minimum of 2 years of experience in medical billing, health records, clinical documentation, or a related healthcare field.
  • Strong understanding of medical terminology, coding systems, and insurance requirements.
  • Excellent attention to detail, organizational skills, and ability to identify documentation discrepancies.
  • Proficiency in Microsoft Office, including Excel, and experience using medical billing or electronic health record software.
  • Ability to communicate effectively and collaborate with clinical and administrative staff.
  • Experience identifying documentation trends and developing strategies to reduce billing errors, denials, and takebacks.
Preferred qualifications:
  • Associate or bachelor’s degree in health information management, Medical Billing and Coding, or a related field.
  • Professional certification in medical billing, coding, or health information management, such as CPC, CCS, RHIT, or RHIA.
  • Experience working with Medicaid and state behavioral health program requirements.
  • Experience with commercial and government payer documentation and billing requirements.
