Position

Home Health Visit Nurse (RN or LPN)

Location: Northeast Ohio (Trumbull, Mahoning, Stark counties and surrounding areas)

Responsibilities
  • Provide skilled nursing visits as directed by the RN Case Manager and in accordance with the care plan.
  • Communicate status/condition changes to the RN and document in the clinical record per agency guidelines.
  • Perform nursing tasks within the scope of practice, including medication monitoring and administration, client teaching, emergency intervention, and other duties delegated by the Case Manager.
  • Recognize and report changes in client condition to the Case Manager and physician as directed.
  • Ensure timely, accurate documentation and maintain client confidentiality.
  • Promote safety and infection control; operate equipment safely and in compliance with policies.
  • Participate in development, implementation, and evaluation of the Agency Quality Improvement Program as needed.
Qualifications
  • Graduate of an accredited school of professional nursing.
  • Active Ohio LPN or RN license in good standing.
  • Minimum 1 year of nursing experience; 1+ year in home health is preferred. Additional home health experience or familiarity is highly desired.
  • Active CPR and First Aid certificates are highly desirable (agency can provide recertification if required).
  • Minimum of 1 year direct patient experience with vent care is required.
  • Strong interpersonal, written, and verbal communication skills.
  • Reliable, professional, compassionate, detail-oriented, and accountable with a good attendance record.
  • Current driver’s license or state  reliable transportation.
  • Willingness to pass state and federal background checks.
Employment Type

& Schedule
  • Long-term employment for Full-time or Part-time positions.
  • No on-call requirements and no overnight shifts required.
  • No required weekends (optional when available).
  • Overtime, respite, and PRN services are offered when available but not required.
Location & Travel
  • Travel within the specified region is required. Part-time roles have a defined travel radius; full-time roles may cover a broader area with travel up to 2 hours and additional cases as needed.
  • Qualified applicants may reside in or near any primary service area as long as travel requirements are fulfilled.
Starting Pay
  • LPN: $28 - $32 per hour
  • RN: $35 - $40 per hour

Position Context

Position reports to the RN Case Manager and the Clinical Administrator/Director of Nursing. The LPN/RN delivers care as delegated by the RN Case Manager.

Essential Functions / Areas of Accountability
  • Provide professional nursing care under the direction of a Registered Nurse and document as required.
  • Perform skilled nursing visits and follow the care plan.
  • Document changes in patient condition and communicate promptly to the Case Manager and physician when directed.
  • Maintain safety, adhere to infection control practices, and ensure quality patient care.
  • Demonstrate safe practice with equipment and participate in safety improvements.
  • Comply with agency policies, state/federal laws, and attend required meetings.
  • Maintain patient confidentiality and protect sensitive information.
  • Participate in Quality Improvement activities and perform other related duties as needed.
Essential Physical Demands
  • Frequent standing, walking, bending, and stooping; occasional sitting.
  • Weight handling: occasional 10–20 lbs; occasional up to 30 lbs; carry 5–30 lbs as needed.
