Home Health RN Visit Nurse

Long‑term employment for full‑time or part‑time positions. No on‑call, overnight or required weekend shifts. Overtime, respite and PRN services may be offered but are not required.

• Monday‑Friday weekdays, 8:00 a.m. – 5:00/5:30 p.m.
• Part‑time: travel radius of Mahoning and Trumbull Counties and surrounding areas up to 60 minutes (occasionally 90 minutes).
• Full‑time:
  Mahoning, Trumbull, Stark, Ashtabula, Columbiana, Portage, and Carroll counties, up to a 2‑hour travel radius.

RNs: $35 – $40 per hour (based on qualifications and experience).

• Autonomy in a client‑centered culture with quality and compliance support and training.
• Weekly pay on Fridays, including onboarding, training, and an additional 4 hours on the first paycheck.

• Graduate of an accredited school of professional nursing.
• Active Ohio RN license in good standing.
• At least 1 year in home health industry plus 1 year of nursing experience; additional home‑health nursing experience is highly desired.
• Active CPR and first‑aid certificates highly desired.
• Minimum 1 year of direct patient experience with ventilator care.
• Good interpersonal communication skills (written and verbal).
• Reliable, honest, professional, compassionate, detail‑oriented, accountable, friendly, respectful.
• Current driver’s license or state  reliable transportation.
• Excellent attendance record.
• Ability to pass state and federal background check.

A successful RN Visit Nurse performs skilled nursing functions under the direction of a Registered Nurse and communicates status changes to the RN Case Manager. The RN delivers care as delegated by the RN Case Manager and follows the established care plan.

• Provide professional nursing care under the direction of a Registered Nurse and communicate status changes.
• Perform skilled nursing visits as directed and follow the care plan.
• Recognize and report changes in client condition to the Case Manager and physician.
• Communicate information in a timely manner and maintain accurate clinical documentation.
• Perform personal care services, assessments, medication monitoring and administration, client teaching, emergency intervention, and nursing services within professional scope.
• Use venipuncture if licensed as “LPN M‑IV.”
• Promote personal safety and a safe environment.
• Adhere to safety and infection control practices.
• Make recommendations to improve safety.
• Comply with agency policies, professional standards, and state/federal laws.
• Attend required meetings and maintain confidentiality.
• Participate in the agency’s quality improvement program.
• Perform additional duties as necessary.

• Frequent standing, walking, bending, stooping, occasional sitting.
• Ability to push/pull 10‑20 lbs occasionally, 20‑30 lbs infrequently; lift 5‑10 lbs occasionally, 10‑30 lbs infrequently.
