Job Requirements

  • Active Ohio RN in good standing is required.
  • At least 1 year of paid experience at a home health agency (or equivalent) is required; 2–5 years’ experience preferred.
  • 1+ years of experience in Case Management, wound care, PICC/PORT/IV Care, lab draws, plus OASIS and Non‑OASIS assessments;
    Nurse Supervision experience is a plus.
Schedule & Service Area
  • Part‑Time or PRN Visit Nurse; minimum travel radius includes Stark, Mahoning, and Trumbull Counties and surrounding areas up to 60 minutes (up to 90 minutes on occasion).
  • Required schedule availability:
    Monday‑Friday (weekdays) 8 am‑5/6 pm. Daytime Visits (No PDN).
  • No on‑call, overnight shifts, or required weekends (optional when available).
  • Qualified applicants may reside in or near any primary service area as long as travel requirements are met.
Employment Type
  • Long‑term employment for Part‑Time or PRN positions; opportunity to expand into larger Part‑Time or Full‑Time employment may be available.
  • Overtime, respite and PRN services offered when available but not required.
Starting Pay Rate

RNs: $35‑$40 per hour.

Qualifications
  • Graduate of an accredited professional nursing school.
  • Active RN with current Ohio license required.
  • At least 1 year in home health industry and 1 year nursing experience; additional home health nursing experience/familiarity highly desired.
  • Active CPR and First Aid certificates desirable; agency will provide recertification if required.
  • Minimum 1 year direct patient experience with ventilator care required.
  • Good interpersonal communication skills—written and verbal.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail‑oriented, accountable, friendly and accommodating, respectful.
  • Current driver’s license or state  reliable transportation required.
  • Excellent attendance record with past employers.
  • Ability to meet all basic job requirements.
  • Willingness and ability to pass state and federal background check.
Position Summary

A successful RN Visit Nurse at Home Care Network, Inc. performs skilled nursing functions in accordance with federal, state, and local laws, and agency policies, and within the guidelines of the professional organization and agency. The RN delivers care to clients as delegated by the RN case manager.

Reports To

RN Case Manager;
Clinical Administrator/Director of Nursing.

Essential Functions/Areas of Accountability
  • Provides professional nursing care under direction of a registered nurse and communicates status changes to the RN per agency guidelines.
  • Provides skilled nursing visits as directed by the RN case manager and follows the established care plan.
  • Recognizes and reports changes in client condition to the case manager and physician as directed.
  • Communicates information in a timely manner, consistent with agency policy and urgency of the situation, verbally as needed and through complete and accurate documentation.
  • Scope of practice includes personal care services, assessment, medication monitoring and administration, client teaching, emergency intervention, and nursing services within professional scope of practice and as delegated by the case manager.
