Key Responsibilities:

• Conduct comprehensive nursing assessments within 8 hours of patient admission to develop individualized care plans.
• Collaborate with patients, families, and interdisciplinary teams to identify teaching needs and healthcare goals.
• Participate actively in rehabilitation programs, delivering direct patient care that emphasizes education and positive encouragement.
• Maintain accurate, timely documentation of assessments, interventions, and patient responses per hospital policy.
• Initiate discharge planning and patient teaching from admission to prepare for transition of care.
• Communicate promptly and clearly all changes in patient status to care team members, including physicians and charge nurses.
• Provide leadership, supervision, and training to ancillary nursing personnel, utilizing effective delegation and decision-making.
• Ensure personal and environmental safety by adhering to hospital policies and demonstrating safety awareness.
• Contribute to a positive work environment through team collaboration and effective communication.
• Manage clinical supplies and equipment needs, promoting cost-effective care and quality outcomes.
• Assume charge nursing responsibilities following orientation, as needed.
• Pursue ongoing professional growth through education and self-study.
