** Responsibilities:*
* * Provide direct patient care interventions, including administering medications, monitoring vital signs, and performing wound care and treatments
* Develop and implement individualized care plans that promote the physical, mental, and psychosocial well-being of our residents
* Collaborate with the interdisciplinary care team to ensure seamless coordination of care and efficient delivery of services
* Participate in quality improvement initiatives to advocate for resident-centered care and improve overall care outcomes
* Maintain accurate and comprehensive records of patient care and treatment
* Communicate effectively with residents, families, and healthcare team members to ensure excellent customer service and care coordination
** Requirements:*
* * Current LPN license in the state of Missouri
* High school diploma or equivalent required; LPN diploma or degree preferred
* Ability to work variable shifts, including evenings, weekends, and holidays; flexible scheduling may be required
* Strong communication and interpersonal skills; ability to work effectively as part of a team
* Familiarity with electronic medical records and basic computer skills
* Compassionate and empathetic approach to patient care; commitment to resident-centered care
