• Assists in development, review, and revision of the plan of care.
• Provides nursing care in accordance with physician’s plan of treatment and the individual plan of care as authorized by the client’s payer source.
• Observes, records, and reports reactions to treatment and any changes in the client’s condition to appropriate personnel and/or physician.
• Provides ongoing instruction and supervision of the client, family, or other health team personnel in the plan of treatment.
• Monitors and records the patient’s condition, documenting provided care services via electronic charting.
• Participates in education programs, quality assessment activities, nurse meetings, and other staff meetings as required.

• Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license.
• Problem‑solving skills and ability to multi‑task.
• Excellent communication skills.
