• Provide direct patient care interventions, including administering medications, monitoring vital signs, and performing wound care and treatments
• Develop and implement individualized care plans that promote the physical, mental, and psychosocial well‑being of our residents
• Collaborate with the interdisciplinary care team to ensure seamless coordination of care and efficient delivery of services
• Participate in quality improvement initiatives to advocate for resident‑centered care and improve overall care outcomes
• Maintain accurate and comprehensive records of patient care and treatment
• Communicate effectively with residents, families, and healthcare team members to ensure excellent customer service and care coordination

• Current LPN license in the state of Missouri
• High school diploma or equivalent required; LPN diploma or degree preferred
• Ability to work variable shifts, including evenings, weekends, and holidays; flexible scheduling may be required
• Strong communication and interpersonal skills; ability to work effectively as part of a team
• Familiarity with electronic medical records and basic computer skills
• Compassionate and empathetic approach to patient care; commitment to resident‑centered care
