The Weekend Nurse Coordinator is responsible for the assessment, coordination, and delivery of skilled nursing care to Suburban Home Health patients. This role provides nursing services in accordance with each adult and geriatric patient's individualized plan of care, including comprehensive assessments, ongoing monitoring of home health services, and education for patients, caregivers, and family members.
The Weekend Nurse Coordinator also oversees and monitors all patients on their assigned caseload to ensure appropriate utilization of home health services and high-quality patient outcomes.
Responsibilities- Complete initial and ongoing patient assessments to develop and update individualized home health plans of care in collaboration with the physician.
- Coordinate and deliver skilled nursing services in accordance with each patient's plan of care.
- Consider patients' physical, psychological, and socioeconomic needs when developing and implementing treatment plans.
- Monitor patients' homebound status and evaluate the ongoing need for home health services.
- Oversee and manage an assigned caseload, making revisions to the plan of care as patient needs change.
- Administer nursing procedures and treatments as ordered by the physician to support optimal patient outcomes.
- Provide education and training to patients, caregivers, and family members regarding medical conditions, treatment plans, medications, and coordination of care.
- Maintain effective communication with physicians and the interdisciplinary care team to ensure coordinated, patient-centered care.
- Supervise visit nurses and home health aides in accordance with regulatory and agency requirements.
- Ensure accurate, complete, and timely clinical documentation.
- Collaborate with physicians and clinicians to promote quality outcomes and continuity of care.
- Participate in on-call responsibilities for assigned service areas.
- Attend nursing meetings, case conferences, educational in-services, and quarterly all-staff meetings.
- Associate’s degree in Nursing from an accredited college/university required;
Bachelor’s degree in Nursing preferred. - Current, active license from the Indiana State Board of Nursing required.
- Minimum of one (1) year nursing experience required.
- Previous experience in home health preferred.
- Prior home health case management experience preferred.
- Strong verbal and written communication skills required.
- Ability to prioritize workload and demonstrate effective time management skills.
- Valid driver’s license and current automobile insurance required.
