Licensed Practical Nurse

Trilogy Health Services is seeking a compassionate Licensed Practical Nurse to join our Healthcare & Medical Services team. In this role, you'll provide direct nursing care to seniors in a long-term and assisted living setting, including medication administration, treatments, monitoring conditions, and accurate documentation. You'll collaborate closely with RNs, physicians, and caregivers to support individualized care plans. Responsibilities

• Provide direct nursing care to residents following individualized care plans
• Administer medications and treatments accurately and safely
• Monitor and document residents' conditions, vital signs, and responses to care
• Communicate changes in resident status to RNs, physicians, and families
• Support admissions, discharges, and transfers as needed
• Ensure compliance with infection control, safety, and regulatory standards
• Supervise and support nursing assistants and other caregivers
• Maintain accurate, timely electronic medical records
• Participate in care conferences and interdisciplinary team meetings
• Contribute to a positive, compassionate, and teamwork-focused environment
