Gentiva is seeking a dedicated Hospice RN / RN Case Manager to provide personalized, in-home nursing care and coordinate with an interdisciplinary team. You will lead assessments, implement plans of care, and support patients and families with education and bereavement guidance.
The role requires an active RN license, 1 year of hospice or home health experience, CPR, and a valid driver’s license. You’ll work in patients’ homes or other care settings across our network.
