If Intern eligible, upload an Intern Eligibility Letter AND subject matter competence: CSET, degree major or coursework evaluation.

If currently enrolled in a credential program, upload a status letter indicating your estimated completion date AND subject matter competence: CSET, degree major, or coursework evaluation. Provide passing assessment scores and/or proof of registration for the Literacy Performance Assessment and/or TPA, as applicable.

Transcripts:
Please upload transcripts showing completion of a baccalaureate or higher degree from an accredited college or university.

Letters of Recommendation:
Three (3) professional letters of recommendation signed and dated within the last 3 years. One letter of recommendation must be from a Supervisor.

Complete Application:
Employment History must Include: employer, dates, supervisor, phone and your job title (If you do not have prior paid job experience, you may include student teaching, field experience and volunteer work.)

Three (3) Professional References. One reference listed must be a Supervisor. Please include name, organization, phone number, title and email of professional references listed.
